Death Certificate - How to Get a Certified Copy

 

There are 3 ways to submit an application to obtain a certified copy of a Death Certificate:

 

  • In person - we are open from 8:30 a.m. to 4:00 p.m. 
  • Online - Note that our office may contact you once we receive your online submission. You must provide a good daytime phone number and email address. If you choose to have your certificate mailed to you, we will send you an email regarding payment. Click here to submit an online application.

Please note that if you are the decedent's surviving spouse or next of kin (father, mother, daughter, son or grandchild) and are requesting a certificate with the social security number on it, you must provide a copy of a government-issued photo identification.

Each certified copy is $20. For payment, we accept cash, checks made out to the Milford City Clerk's Office, money orders and credit cards. Please note that for all credit card transactions, there is a $1.50 convenience fee.
